Print, fill in, sign with a pen and mail in the Australia Post; 'Keep the tracking number' and track envelope to it's destination of;

If sixty years old or older, please post a medical certificate with these papers signed by your GP.
If NOT completed correctly, You will not be able to attend the hike.
It's your responsibility to ensure forms and medical certificate are completed immediately after making payment for the hike and post in the Australian Post with a tracking number and ensure it arrives to it's destination. All three links are below here for printing. You may choose to take Travel Insurance out on the day of booking in with HIKE WITH MAREE to claim refund in case your can't make the hike due to unforeseen circumstances occuring.
